FIREARMS PERMITS & FEES

What follows are the necessary forms and fees that must be submitted when a person requests a Firearms Purchaser Identification card or a Permit to Purchase a Handgun. The forms may be obtained from the New Jersey State Police website at: http://www.njsp.org/info/forms.ht

  APPLICANT HAS NEVER HAD A FIREARMS ID CARD:

• Application for Firearms Purchaser Identification Card and/or Handgun Purchase Permit (form S.T.S. 033)
• Consent for Mental Health Records Search (form S.P. 66). This MUST be signed in a police officer’s presence.
• MorphoTrak Universal Fingerprinting Form (form NJAPS2). The applicant must contact MorphoTrak to schedule an appointment to be fingerprinted. The contact information and instructions are included on this form. There is an $11 fee payable to MorphoTrak as well as a $60.25 fee payable to the NJSP SBI for this service.
• A $5.00 fee for the Firearms ID card and $2.00 per Pistol Purchase Permit, if any, must be collected.

  APPLICANT HAS A FIREARMS ID CARD AND IS REQUESTING A PERMIT TO PURCHASE A HANDGUN:

• Application for Firearms Purchaser Identification Card and/or Handgun Purchase Permit (form S.T.S. 033)
• Consent for Mental Health Records Search (form S.P. 66). This MUST be signed in a police officer’s presence.
• Request for Criminal History Record Information for a Non-Criminal Justice Purpose (form SBI 212A). This must be submitted with a money order or certified check in the amount of $18.00 made payable to the NJSP SBI. *** This may be waived. Please see the Firearms permit officer.

• A photocopy of the applicant’s Firearms Purchaser Identification Card must be submitted.
• A $2.00 fee per Pistol Purchase Permit must be collected.

  REPLACEMENT OF A LOST FIREARMS ID CARD OR A REQUEST FOR A CHANGE OF ADDRESS:

• Application for Firearms Purchaser Identification Card and/or Handgun Purchase Permit (form S.T.S. 033)
• Request for Criminal History Record Information for a Non-Criminal Justice Purpose (form SBI 212A). This must be submitted with a money order or certified check in the amount of $18.00 made payable to the NJSP SBI. *** This may be waived. Please see the Firearms permit officer.
• Consent for Mental Health Records Search (form S.P. 66). This MUST be signed in a police officer’s presence.
• A photocopy of the applicant’s Firearms Purchaser Identification Card, if this is not a request to replace a stolen or lost card, must be submitted.
